Introduction

The Healthy Mind Consultancy take the privacy and security of your personal data very seriously. This Privacy Policy informs you about how I collect, use, share, and protect your personal information when you connect with my services. It is important that I have your trust, and I am committed to safeguarding your data to ensure transparency in how I manage it. 

The type of personal information I collect

 

I currently collect and process the following information and it is added to my secure Database only when we have a booked an appointment.  My database is called Zandahealth and you can find the security info here: zandahealth.com

 

• Personal contact details such as name, title, address, telephone numbers and personal email addresses

• Date of birth

• Marital status, dependants, etc

• Next of kin and emergency contact information

• Information about your race or ethnicity, religious beliefs and sexual orientation.

• Information about your physical and mental health, including any medical condition and medication

• Information about your income level to establish the cost of counselling

How I get your personal information and why I have it

 

Most of the personal information I process is provided to me directly by you to provide a service to you. 

Specifically, I use the information that you have given me to provide a Psychotherapy service or allied service.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

• Processing of your data is necessary for a contract I have with you or because you have asked me to take specific steps for you before entering into a contract.

• Processing is necessary for The Healthy Mind Consultancy legitimate interests, unless there is a good reason to protect the individual’s personal data which overrides those legitimate interests.

How I store and protect your personal information

 

Your information is held securely either within the Healthy Mind Consultancy secure system called Zandahealth.  You can find more information on its security on the website above. 

I keep all personal information held about you for a period of seven years from the end of my services to you before I then dispose of your information by confidentially disposing of it or deleting it from my computer server.  You are welcome to request in writing that it is deleted before this time. 

 

Your data protection rights

 

Under data protection law, you have rights including:

• Your right of access – the right to ask me for a copy of your personal information.

• Your right to rectification – the right to ask me to rectify inaccurate personal information. You also have the right to ask me to complete information you believe to be incomplete.

• Your right to erasure – the right to ask me to delete your personal information in certain circumstances.

• Your right to restriction of processing – the right to ask me to limit the processing of your personal information in certain circumstances.

• Your right to object to processing – the right to object to the processing of your personal information in certain circumstances.

• Your right to data portability – the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.

If you have any concerns about my use of your personal information, you can make a complaint by emailing thehealthymindtherapist@gmail.com with ‘complaint’ clearly written in the subject heading.

You can also complain to the ICO if you are unhappy with how I have used your data.
